FLR Application while on WP doubts!!

Switch from WP to HSMP through FLR some doubts!!!
I am presently on WP since past 10 months. I get salary in india and allowance as well in india which I withdraw here and use for my expenses here. My HSMP has been approved (indian salary and payslips) and I wish to make FLR application while staying in UK. I have the following doubts which I need to clarify regarding making the FLR application

1. For accomodation my company has provided me with a 2 bedroom apartment. The lease etc is with my company. So what proof can I show here for accomodation? Should I just mention company provided accomodation in the application?

2. Is it fine if I show my indian bank account which is in INR for funds proof or a UK bank account is needed ? Do they expect a UK bank account since its a switch from WP to FLR within the UK?

3. There is a question "Are you working in the UK? If so what is your net salaryy?" What do I need to put here as I have no UK salary. Also I did not claim any points for working in UK while making HSMP application

Please help me on this. Can someone provide a covering letter format as well for making my FLR application.
