What do you mean same job title ?
In the form, it is up to you to define the Job Title. So it can Waitress 1 and Waitress 2.
It may become a problem if the 4 staff were hired for 6 months each, one after the other. But if you had 2 people working at the same time for a good part of your claim, then it can be quite safely argued as two jobs.
