Schengen visa - Affidavit for name change of spouse

I am an Indian citizen (Indian passport) working in UK on ICT visa type. I plan to apply for Schengen visa from UK for me and my family. My wife is dependent (not working). My marriage certificate (issued in India) has my wife's maiden name, and her passport has post marriage name. I was told that I would need an affidavit stating that both names are of one and the same person. Is this a requirement to apply for Schengen visa?
