NCS - Joint application for BN and Passport - what form?

Sorry to be a pain but do I have to fill in an application form for the passport prior to my meeting with NCS? If so what is the application called for EU citizens? My local NCS offers joint applications for BN and passport at the same time and I want to make sure that I have everything ready.

Form AN for citizenship.

Passport form you get at the Post Office for the passport.

No 'special' forms for EU citizens.
