A1 English test / spouse visa

HI All,

my wife got the visa from Pakistan as spouse/partner visa 14/09/2015 to 09/06/2018 total she got 2 years ,8 months 27 day , visa

now we need to apply fro extension for her visa,
Q1. can she re use her A1 qualification for this extension which we show on first visa,
Q2. she got masters degree in English can i certificated it from NARIC and use it ?
Q3. which visa application should i fill i suppose its FLRM and can i do online,
Q4. HOW MUCH I SHOULD PAY FOR HER NHS FEE as last time iv paid £600 but we didn't get visa for 3 years?

Q2. she got masters degree in English can i certificated it from NARIC and use it ? Yes with a NARIC ELA (red route)

Q4. HOW MUCH I SHOULD PAY FOR HER NHS FEE as last time iv paid £600 but we didn't get visa for 3 years? Currently £500, but due to double at some unconfirmed date later this year.
