Out Of Country Tier 2 General Visa Application Process and General Queries

Hi,
Earlier tier2 general visa application URL was: https://www.visa4uk.fco.gov.uk
Recently Noticed that it Out of country application is being redirected to: https://visas-immigration.service.gov.u ... eneral-ooc

Also, earlier URL (https://www.visa4uk.fco.gov.uk) says "The official UK website for online visa applications, for applicants not in the United Kingdom."
Currently, I am in the UK dependant (ICT) Visa for last 2 years and I need to apply Tier 2 General Visa from India. Here are my queries for which I am looking for some assistance.
1. Which URL should I use for online application (visa4uk or visas-immigration.service).
2. In the Application, there is a question regarding Medical treatment "Have you ever been given medical treatment in the UK?" - During in current staying, I have consulted with Doctor in GP to get the medicine prescription and also my baby is born here in the UK. Please suggest In this scenario should I answer this question as Yes or No.
