Tier 2 general visa application doubts

Hi All,

I have couple of basic doubts related to the application form, shall be thankful for any help.

1. Could anyone please confirm which is the correct website for visa application?

2. There is one question related to medical treatment asking about the medical treatment received in the UK or not. I was in the UK on ICT visa previously and had been visiting to GP surgeries and visited one private doctor in a hospital (paid by employer insurance). So do I need to mention all the GP surgeries visits or just the hospital one? Also for the hospital visit what should be the answer for follow up question: Did you pay for the treatment? It was a private appointment paid by insurance.

Shall be highly thankful for any help.

Regarding 1) I was having similar confusion which site to use.

I have called and confirmed from help centre of Home office(paid service). They suggested me to use Visa4UK site. I am filling form in this site now.
