British passport application: Grandparents' details needed?

Hi Guys,

My wife got naturalised last week, so I now want to her passport sorted out.

i) do I still need to complete section about her parents? Grandparents as well?
ii) do I need to provide birth certificate?
iii) are there any documents to provide in addition to the naturalisation certificate?
