Tier 2 Dependent extension, main applicant on ILR

Wonder if you be able to help


My husband granted ILR in Sept 2018 and I am currently on PBS Dependant which will expired in April 2019. I wish to apply extension of my PBS to complete 5 years on my own.

1) kindly provide link for online application?

2) will I subject to an increase IHS fees even for extension not 1st time application?

3) proof of maintenance - shall my husband and I provide bank statements or my husband’s statement will suffice?

4) will 1 utility bill + bank statements adequate to proof co-inhabitant?
