self employment (Appendix 2)

Hi ,

Thank you very much for support, Great source of information. Quick question please,

As my self-employment started in Aug 2018 & previous 2 years was on permanent job,
In Appendix2 , Im on Cat F and need to fill Part 3D. As it says other non- employment source , bit confused.

Is self employment is considered as non-employment?
Did anyone complete Part 3D (Appendix2) for self employment?
Im not sure what to fill in section.
In Sec 3.61 a),source of Income : I believe company details who gave contract isn't?
In Sec 3.61 C), Account/Reference details: is this Accountant details?
Do I have to fill Sec 3.62 & 3.63 for self employment ?

Please advise or share you experience.
