Decision made but delay in passport collection

Hello,
How long does it take to get the passport back from VFS after the decision is made (called HO and they said its been decided)?

I know it might depend on location but any answer would be appreciated especially from people in europe (out of UK applicants).

Additionally, does that mean its refused as it usually take 1 to 3 days to receive the passport after the decisions been made?
