Has refugee travel document holders traveled to any of the Caribbean islands without visa? Or visa on arrival?

If you are a visa free national to them then yes you can. Your nationality usually determine that.

Yes I m aware. There are only few EU countries which allow visa free entry on Refugee Travel Document (blue) issued under the 1951 UN Convention. However, if any chance if op has or will get his country passport then may travel to more countries or island without visa as listed in previous link.
