Why are reading and posting a document link that has absolutely NO relevance to your immigration route? The link you posted is ONLY relevant to EU settlement scheme applicants, or EU citizens and their family. Employer’s address can be googled. Dates can be estimates if exact not known. Clarify in the cover letter if need be.
Employment is not necessary for AN application. One could even be unemployed for last 10 years. However, you have to mention (if any) employment in UK for last 10 years. Gaps are irrelevant.
