Naturalisation Application - Waiver request (KOLL form)

Hi,
I am submitting an application for someone whose GP has provided the applicant with a Waiver request for the knowledge of language and life in the UK requirement. (KOLL Version 11/2018). When filling out the form online it asks for the following;

1. Applicant's completed English language medical exemption request form
2. Applicant's completed Life in the UK medical exemption request form

but isn't this the same form? do I need to make two copies of it and submit them?
