Observations on the first UK passport

Hello,
Has anyone ever seen such a note on a British passport?
I've read about countless of people who applied for their first UK passport after naturalisation using their foreign passport. However, this is the first time I have seen someone getting official observations on their UK passport with the data of their foreign passport.
Does anyone know when this happens?

Does your VN passport show a different name from your UK one?
I believe that may happen when HMPO accept that the names can't be the same, due to the rules of the other country.

It is not my passport. I've asked for an acquaintance of mine. I also think that is the issue.

Do remind your acquaintance to sign the passport, preferably in black ballpoint pen, else it may be invalid.

Also make copies of the bio page to record passport number and issue date, etc.
