University letter format for name difference

does somebody know the letter format from University for a name difference on degree than passport. i have some name difference in masters degree cert and on passport. please share ur thoughts. thanks a million

I think that a simple letter with following wording will be fine.

This is to certify that Mr. XXX (this should be the full name as on the passport), S/o Mr. XXXXX, Date of Birth XXXX was awarded xxx degree in xxx subject on xxxx date by the University after successfully completing the 2 years Masters in xxx course from the xxxx institute.

This is further to certify that Mr. XXX 's name has been noted on the degree certificate as Mr. YYY as per University's convention. A certified copy of the Degree is enclosed with this certificate.

The undersigned can be contacted at the contact number/address provided herein under, should any further information is needed in this regard.

is it enough to get the general affidavit and notary certificate from local notary , saying that all the names refers to the same name.

or do i need to get separate letter from university that the name referred in certificate is the same as in passport.

The problem is - a general affidavit and notary certificate is an undertaking that you are giving as opposed to the letter from University which obviously carries a lot weight.

@ push_hsmp -to understand clearly, the general affidavit and notary certificate from local notary is sufficent and considered as valid evidence compared to letter from university.
