The letter should preferably on the institute's headed paper and signed and stamped. It should cover your name the dates you studied at the institute and the course you undertook. It would be useful to confirm if the course is full-time or part-time and what qualification was attained. Also, the letter should state that the course was fully taught in English.

That said, if you look clearly at the guidance, the letter will need to be in the form of an original academic transcript.

Dear Sir/Madam,

This is to confirm that vineet.b5 was a student at the XYZ University between date a and date b. He/She completed a full time course in Underwater Basket Weaving and graduated on date c with a Bachelor's degree in the above. The course, including all lectures, and underwater basket weaving practical sessions were taught entirely in English.

Please do not hesitate to contact us for further details.

Yours sincerely,

If the Self assesment tool awards you the 10 points for English section, then you do not need a NARIC letter, however if it does not then you do require a NARIC letter.
