Help on Permanent Residency

I am in UK on 2 yrs work permit and 3 yrs HSMP.
I am now applying for PR. I need help on below.

1. What docs are required to prove economic activity for 5 yrs.
2. Any docs required from spouse ?

has anyone recently got PR while on HSMP. Can you provide list of docs submitted ?
