URGENT:Tier1 Dependepnt visa queries

I got my In country WP->Tier1 Visa yesterday.
I am planning to apply for my dependents since i missed their applications with main application(which i was not aware).

I have 2 dependents(spouse+ 4 years old kid ).

Dear members,

Could you please clarify below queries.

1.I hope i need to pay one fee only (£395) where as application fee for my kid(4 years) is not required.
2.Do i need to send my passport along with dependent applications? or photo copies of my passport.
3.For maintenance of funds i am planning to send my bank statements?Do i need to take a latter from bank also stating that i have
enough funds from last 3 months.
4.Can i pay the application fee my credit card (since i am not applicant)
5.Documents i am planning to send is
A.2 filled applications , B.Photos C.3 months bank statements D.Marriage and birth certificates E.My passport ? F.ANY THING ELSE?

I didn't get clear information from application form.
