LTR Application Procedure

Hello,

I am applying for LTR and got the information of the application procedure from INIS. I kindly request a format of the letter which has to be sent along with other documents (Passport copies, Work Permit Copies, GNIB Card Copy) from anyone who had applied before. It would be a great help. And also pls let me know if I am missing anything in the list for the required documents etc, Do these documents need to be attested by a solicitor?
