Name confusion

All my education certificates have only my name and surname . But my Passport has my Fathers' name included as Middle name .
What name should i use now in my HSMP form ??!!! I mean should i include my fathers' name in the "First Names" of the Personal details section in the HMP form

I think you should use your "passport name", and not worry at all that some of your certificates omit your middle name.

Presumably your passport matches your birth certificate?

Hi,
Don't worry about name, just write your passport name(s) and last name as Sir name.
