Where to Mention the previous visa refusal details?

Where to Mention the previous visa refusal details?

Hi, My IGS to PSW visa was refused a month before and the reason was my bank statement was 5 days short for the 3 months .They refused on the basis of this and they mentioned it on the refusal letter. Now I am applying a fresh application and I cant find any section in the PSW form asking for previous visa refusal details,but some of our mates here advised me to mention it on the new application about my previous refusal.

SO where can I give the details of my previous refusal? Should I have to make a cover letter and mention it?
