What visa application form for dependant of old work permit

Hi
my friend is working in the UK as a chef. He arrived here before the tier sytstem came into effect and is a Work Permit holder. If his wife wants to come and join him in the UK what visa application form does she need to use. I've had a look on the UKBA site and just find dependant of PSb forms.
