applying for extension: Maternity break inbetween, help pls

Hi,
I am applying for extension of my hsmp next month. But I had taken 6 months maternity break(8 months ago) and returned to full time work.
Now to show my 12 month earnings I plan to show 4 months payslip before my maternity leave and 8 months payslips after maternity leave.
Will this be alright? And what evidence should i provide to show that I was on maternity leave during those 6 months?

were u paid during maternity leave...

Yes I was paid Statutory maternity pay during this period.

You need to provide maternity leave letter from employer OR you can show child's birth certificate and SMP payment (Salary slips).
