I have recently got my extension approval so trying to help you based on my experience.

About B15-B17, you need to list down all the addresses at which you have stayed in the last 2 years. In my application I gave addresses of a rented house which was a long term accomodation, a short term serviced apartment and my indian address too.

I only have a mobile, no landline so I gave my mobile number.

B10: Yes, HO Reference number is the same as mentioned in your HSMP reference letter.

B12: This is a bit confusing, are you applying for an externsion or is this is your first application?
If extension, then you have had some earnings in UK which means u must have got a NI number.
Please calrify.

UK card number is not mandatory, I too dont have one.

Tier1 Gneeral application form Help says -
B13 : An applicant who has made previous applications under the points based system will have been given a points based system Migrant Reference number. This allows us to track and link past applications. The applicant
should provide his/her full points based system Migrant Reference number. This number can be found on any previous correspondence relating to applications under the points based system.
