Tier 1: marriage certificate and online appointment

Hi guys,

I am in final stages to apply for Tier 1 for me and my dependents, two concerns I have:

1- One is about marriage certificate. It was issued after my marriage, four years back. Can I use the same one or should I apply for a fresh one?

2- For applying Tier 1 with dependents online, should I make one online appointment for all of us or separate for me and each of my dependents?
