Tier 1 extension form questions

Hi,

I am applying for tier 1 extension with dependents very soon, already under hsmp.

I have some queries:

1. In main applicant application form, section 6, summary sheet, what to fill in "documents provided(please list)" for uk experience, english language requirements and qualifications. Should i just put, refer to cover letter.

2. D11 in dependent application, what should be written for child and wife?

3. Section 5, Declarations, in dependent extension application, for a child aged 1 year what should i put in the name and signature field
?

4. For sending the application as registered signed for using royal mail, do i have to send it to postal address or courier address?

You have to follow HO rules not the other way... Fill in the application form..

2. D11 in dependent application, what should be written for child and wife?
HSMP Dependent Visa or what ever is written on ur depend's visa at TYPE

3. Section 5, Declarations, in dependent extension application, for a child aged 1 year what should i put in the name and signature field
?

NAME: Put ur KID's Name and
SIGN: you sign and in bracket you can put (For mention your kids name)

4. For sending the application as registered signed for using royal mail, do i have to send it to postal address or courier address?

POSTAL ADDRESS, HO has arrangement with RM for all ordinary and registered signed posts..

There is no harm sending it along with ur application.. i would advice to attach the PBS Summary sheet and send it with your application which acts like a checklist(quick view of ur points claimed) based on the PBS calculator

For documents list, i am writing this in the form, please correct if i am wrong.

uk experience - 12 month salary slips from xyz.
english language requirements - cover letter, Evidence with initial application for hsmp.

qualifications - Evidence with initial application for hsmp and points based calculator

Also in T9 , i have mentioned "refer to cover letter" as it was written that if more space is required, provide in cover letter -- Is that ok?

Also can you provide me with cover letter for main applicant and spouse and child - that would be really great.
