hsmp to tier1 extention fee

Guys

I need your help.
I am about to apply for my extension, could you please let me know 820 is correct? I am on HSMP since Jan-2008, so will apply in Oct some time.
I have three dependents so fee will be like that..

820 + 50 + 50 + 50?

Just to confirm one more question.

If I am submitting documents for my whole family, should I be sending it 4 forms (myself, wife and 2 childern)?

There is a separate form for dependants

If you have dependants who wish to join or remain with you in United Kingdom, you or they should complete the separate dependant application form.

Please be careful with the sections as it is the same form for all sorts of dependants, be it Tier 1 or Tier 5

No there is no harm but they have given specific instruction about one of the pages being printed on the back of the payment details. I'm pretty sure its the same scenario with the dependants form as well. Read carefully and have a look.
