HSMP to Tier1 - Dependent Application Form number!!

HI..
I am applying for tier1 general. Am currently on HSMP.

Which form should i fill for my dependent(spouse). The one i am looking at it PBS(Dependent) VERSION-10/09. But this form doesnt say anything about HSMP ( non judicial review).

Any help plz?

That is the form for all Tier 1-4?? dependents...
