Need help in filling form form for extension HSMP - Tier 1

Hi,
Please help me in filling the application form. My current leave expires on March 7th 2010.

1) Is it enough if i send the extension form before 5 weeks of current leave expiry date ( i,e Last week of Jan 2010)
2) Should application form needs to be filled with Black Ink or Can we write & print in PDF?
3) Section 1 - A5 - Contact Name in the UK if different from that of the applicant- ???
4) Section 2 -B27 - Applicatants main occupation - ???? ( I am in a software developer)
5) Section 2 - C9 - ICFN - ??? ( Since my current leave is on HSMP, i think i should leave C9 empty & fill C10, stating the same reason)
6) Section 2 - D3 - ??? ( I came to UK initially on WP & then moved to in country HSMP) Should i write HSMP Visa issued Date or WP visa issued

7) Section 2 - D4 - ??? ( reference number on the entry clearance) - HSMP reference number or WP reference number
Section 3C - S1 - What is the Grade???

1) Section 4 - J5 - ??? ( Do we need to fill this ) J1-yes,J2-No,J4-No

2) Tier1 General Application Form
3) PBS Dependant Form
4) Passports ( Self + Spouse)
4) 4 Photos ( Self + Spouse)
5) Original Marriage Certificate
6) Earnings covering letter

6) Qualifcation - None [Evidence provided with inital HSMP application]
7) Previous Earnings - Original Payslips(not stamped) + Original Bank Statements + Employer Letter on Company letter head stating earnings for the period claimed + Employer Reference Letter
8)UK earnings - None
9) English - None [Evidence provided with inital HSMP application
]
10)Maintainence( Self + Spouse) - original bank statements for last 3 months with more than 2000 pounds

Can you please copy paste the text of Employer Reference Letter..........i also do not have stamp on my payslips as my company dont have a stamp i have arranged a letter from employer with salary information.............but i was not aware of this reference letter. Please let me know what is the content in the letter so i can get 1

What is your current visa status? HSMP? then you shd choose a extension of existing leave n go to L1
