Please help me in Previous Earnings section.

I joined my employer on Nov 10th 2008. My first salary for period ending Nov 16th 2008 was credited to my bank account on Nov 21st 2008. The bank statement shows the salary was credited on Nov 21st 2008. The payslip shows 2 dates; 1) Period ending date: Nov 16th 2008 and 2) Advice date: Nov 21st 2008.

Please tell me what is my start date (I need to fill that date in appendix)? Options are:
1) Nov 10th 2008 - The day I joined my employer
2) Nov 16th 2008 - Period ending date
2) Nov 21st 2008 - The day I got my salary credited to my bank account


Same is the confusion with end date:
Payslip shows "Period ending date" is May 31st 2009 and "Advice date" is June 5th 2009. Banks statement shows that my last salary was credited on June 5th 2009.

Which date should I use to get the closing exchange rate on OANDA website?

So, in essence I am asking should I use the date on which I last worked OR should I use the date on which my salary was last credited? As you can see from my above explanation there is always a difference of one week.

For Earnings Period..
you should put the dates of the period you worked and got paid..

So in your 1st case since you joined on 10th Nov 2008 this will be your start date of work and pay starting...

In your 2nd case if your last date of working with that company is May 31st 2009, you should use that date.. no matter when it is paid.. few days before or after..
