Tier 1 Dependent application form for New born Baby in UK

Hi Experts,

I have got a slight confusion on which application form do i need to fill in order to apply for my New born baby Visa under tier 1 Dependant category for the first time.

In UKBA web site it suggests the following form for dependents.

but when i open the form it says

Application for an extension of
stay as a dependant of a person
who has leave to remain under the
points based system

My confusion is that do i need to use the extension form even though i am applying for the first time for my baby and she don’t have any leave to remain on her passport?

Is the extension application form and first time application form for the dependant is same ?
