HSMP Ext Tier 1 Self employment

HI

Please anyone can let me know excatly what documents i need from my Accountant to proof my earnings as self employment.

-Letter from accountant with invoice details with gross income
-bank stantement
-balance sheet showing the earnings and my expenditures.

please let me know if i need anyother documents i might need.
My earnings are in 1 feb 2009 to 31 Jan 2010, so will get TAX paying related docs later.

I submitted the following documents for my Tier-1 extension obtained from my accountant:
1. Letter from accountant detailing gross, net salaries and salary credit dates
2. Unaudited financial statements of the limited company for the earnings period claimed showing balance sheet and profit/loss

Apart from these I submitted my payment invoices, business bank account statements and company incorporation documents.
