Signature on dependant form for Child

Signature on dependant form for Child

Hi,

In Dependent application(tier1 ext), do we need to put thump impression of my child (age 4) on the place of signature column or shoul i leave as blank?

Note that PBS form for child is applying along with main application.

You may sign in place of your child (mentioning that you are his/her parent)

Yeah a parent can sign and mention the same. I have done the same for my 2 year old kid.

Should we need to write our name in brackets

Yeah a parent can sign and mention the same. I have done the same for my 2 year old kid.

Yes, and also you can mention (John Doe/Jane Doe, father/Mother of applicant)

Father of applicant

No dont fill page 37. It is only if you or your dependents are being represented by a third party like a solicitor etc.

I think you have referred wrong page.

Page 37 is not for Representative. Rep. page is 41. on PBS form

Please confim...

Sorry my mistake. Page 37 in old application is for representatives and in the new application, it starts from page 38.

In the name part you can just fill the applicant's name and sign in the box and outside the box mention that you are signing on behalf of the applicant.
