Surname change

Hi,

As i had typo in surname on my passport and applied for new with correct one and got the initial HSMP visa on the new passport,but the name chnage is mentioned in the second page of my new passport as "This holder previously knows as XXXXXX".

So do i need to mention this spelling correction in B4 (Name known by) and evidence as my current and old passport???

Yes you got it right..

It is better to fill that part as you got it.. better safe than sorry mate..
