1. Covering letter
2. PBS dependant form (Version 04/10)
3. Visa fees - £475
4. 2 photographs of the baby
5. Baby passport + My passport
6. Original birth certificate
7. Maintenance funds - 3 months bank statements
Anything else?

Ensure you include the following also:
1. Family pictures (Pictures of father, mother & baby)
2. Vaccination booklet
3. Please also include the bio-date & visa page of yourself and your wife)
4. You should also include your bank statement to cover all of you. To show that you can care for the family without recourse to public fund.
5. If you are staying with someone in the UK, get a letter from the person stating that he/she is still able to accommodate you with the baby.
6. Also get the sponsors visa & bio-data page.
I hope this helps. i just did for my baby also and i got it last week.

5,6. I am the Tier1 visa holder and no one is sponsering me, so I guess these points are not valid for me.

Thank you for all the support, today I have received the Tier1 dependant visa for my kid.
I have one doubt: I am having a Tier1 visa which got extended recently for 3 years and my baby also got the visa with the same validity. Do I need to take the biometric tests for my baby? He was born in UK and haven't yet travelled anywhere.

Application sent: 27th April 2010
Fees (£475) deducted: 28th April 2010
Passport received: 14th May 2010
Visa validity: 3 years

Could you please clarify how much do I need to show in my personal bank account for last three months? Is it £533 (only for my baby) or do I need to show £800+£533+533 (myself+my wife+baby)?
Please note that my wife is already on Tier1 Dependant Visa.
