Tier-1 dependent visa for a child and Spouse Name change

Hi,

I am currently residing in UK for last one and half years. My wife was with me as a Tier-1 dependent and her dependent visa was also issued on the passport which carried her maiden surname/family name.

In December 2009, she went back to India to deliver our baby. After child's birth, we applied for baby's passport as well as my wife's passport. Wife's passport was applied to change her surname as well as address to reflect her new family name (i.e. my surname).

My questions are:

1. Since my wife would be applying for our baby's Tier-1 dependent visa application from India, what name she should use while filling up baby's VAF10 form?

2. Whether documents listed below would suffice?

a) Maintenance funds for 3 months ( online statements attested from bank + bank letter confirming funds in account)
b) Child's birth certificate
c) Copy of Tier-1 visa endorsement from my passport
d) Employer letter confirming my employment status
e) Rental agreement of flat in UK as an address proof

Use her current name in her new passport (hoping it is same as it is in ur child's passport )
