Sign on behalf of 1 year old child

I am preparing Tier 1 dependent application for my 1 year old kid. Understand that I have to sign on behalf of child as child is one year old. The confusion is that there are boxes above signature where name has to be put. I am confused whether it should be name of child or parent?
