Expired and renewd passport has different name - urgent

Dear All,

My name in recent Passport is Idris Yunus Halai where as old passport carry Mohmed Idris Mohmed Yunus Halai.

do I need to submit both the passport and if it is then what supporting documents will be required to prove that both the names are same.

What is the name shown in your documentary evidence(s) - payslips, bank statements, qualifications etc.?

Only old Passport whcih is expired on nov-2009 carry name - Mohmed Idris Mohmed yunus

Rest all documents ..new passport, payslip, degree certi, mark sheet, banking has Idris Halai...

Do I need to mention old passport name in application form colum 2.3

Answer all questions truthfully. You were indeed known by a different name earlier ... as shown in your old passport (an identity document).

Suggest that you arrange for the necessary affidavit and submit along with the application.
