Few problems with application form

Hi People,

I am filling my PSW Application form. Had few confusion

1) What is pbs migrant number. I read description but still confused?

2) What is home office reference number/ What to enter?

3) I lost my passport and issued a new one . I went to the branch and embassy of my country here. They reissued my passport from My country and I recieved it here. What will be my place of issue of this passport? My home country or this country

4) What does post(City) of issue means? Is it city name or post code of city?

5) I am a post graduate msc xyz. What should i enter in Qualificaiton field?

MSC Or post graduate?

6). While signing the form and declaration at end they ask specifically about a date? which date is it when i have an appointment or the date when i m filling and signing this form?

5. what is ure qualification ? MSC or PGD. they both are totally different.
or might not get any PSW on PGD.
