wife stay 3 years, can she apply ILR with me?

Hello,
I am on Tier Visa and due to get my ILR on apr 2011.
My wife has completed just 3 years as my dependant.
Can she apply with me on the same form?

Does she have to fill a separate form? or a SET(O) form common for both is allowed?
