Multiple COS requests by the employers

Is it possible for an employer to request more than 1 COS at a time?

For example,

Request #1
with job title "ABC" salary 24k

Request #2
with job title "ABC" salary 28k

Request #3
with job title "ABC" salary 32k

question #2

What if the cos ISSUED BY ukba before 1st April 2011(in March panel) , and assigned by employer before 1st april, is used up by the tier 2 applicant AFTER new rules have come into place?

is it still valid COS?

Question1: My company did like that. (Inc. our subsidiaries) But we couldn't get anything at all. (I feel its our HR's fault though)

Our HR didn't give the reason clearly (please refer to the letter I quoted in other thread) They asked 'additional information' about our application in Dec but finally HO didn't satisfied with that.
