Self Employed or limited company Tier 1 General Extension

hi all....please help me i would be truly grateful.....

Please pardon me if this query has been asked before.

I work as a locum and own a limited company. I have been taking monthly net salary from the company bank account through cheque to my personal account. Also my accountant issues 12 months payslip sthat shows my salary. He also looks after PAYE, tax and NIC.

Now for tier1 (general) extension, is it okay if I send the following documents?:

1. Payslips printouts
2. Personal Bank statements showing monthly net payments

Should I also include letter from accountant?

Do I classify as employee or self employed as I am the only director?

Should I send any other supporting evidence for earnings?

Koalachan, Sushil requires these details to advice if the documents that you have listed will suffice OR not for providing as evidence while including during the extension application.

Seniors, For operating through Limited companies and if you are the only Director do you receive any Payslip from the accountant? Is this really required? please advice.
If the opportunity doesn't knock, build a door.
