ILR on a separate sheet

I was granted ILR in Feb this year. UKBA did not stick the visa on the passport, instead gave it on a separate sheet of paper, on the argument that there was no empty sheet in the passport, even though it had around 4 empty sheets. I was told to get the visa transferred to new passport booklet.

I have managed to get a new booklet now. What is the way to get this transferred to new passport? Do I need to fill the NTL form and submit? Thats another £216 I guess.
