ILR from EEA route

hello all
I have living in UK since 2002. 1st 6yrs as student, 1 yrs PSW, n 2 yrs running as a family member of EEA citizen. currently holds EEA2 that expires in 2016.
My question
1. As i will complete my 10 yrs nxt year, would Ibe able to apply for ILR/PR?
under long residenct act section 2.3.8 i can apply for permanent residence
2. which form i should use SET(0) or EEA4, and do i have to pay any fees
