Lloyds Tsb Statement on page missing

Dear all,
I have submitted bank statements of lloyds tsb with my Psw application.I requested statements from 10th march 2011 to 10th june 2011 but i received statements from 3rd march 2011 to 10th june 2011.(7 pages)
while enclosing these i forgot to enclose the first page which is 3rd march to 10th march.
The statement covers 90 consecutive days but i am not quite sure that whether there is my name and address on the remaining pages of the statement.
I would be grateful if anyone could tell me the structure of Lloyds Tsb statements if 3 months statements is ordered once.
i am looking for advise on what steps should be taken in this situation.

whatever is missing , you print online urself go to lloyds and get it stampd, i did tht too , they have date cycle when printing usual statement, so wotever u received is fine and rest u print online statement and get it stamp. wont have any problem.
