employer letter

Hi,
Could anyone advice me with this:
1) Do i need to get the employer letter from my Boss who had sponsored my work permit in 2006 or from the HR(NHS), cos in the O set form it mentions letter from sponsor.
2) there isnt a exact job match for me on code list,,i assume i have to take one which is closer.
3) I only have a photocopy of my work permit , do i need to certify it.
4) If one of pay slip is missing..does bank statement proof is enough.
