Query on Travel details

This is a slightly complicated situation:

I had travelled on a work permit and got my tier 1 extension while i was in UK , while on tier 1 i travelled back to india for 1 year , and then came back on the same tier 1:

the travel dates are as followed :

5th august 2007 , first arrived in UK on WP
6th Sep 2008 obtained Tier 1 Visa ,
30 November 2008 travelled back to India
6th june 2010 came back to UK on the same tier 1 ,


Now the query is :

under section D (Immigration history) of the tier1 extension for , question D8 :
D1:

Did the Applicant obtain entry clearance or a visa before entering the UK?

( My question is what do i mention here WP or Tier 1? ) ??

D3 What date was the entry clearance or visa issues:

again same question.??

D4 Please enter the reference number on the entry clearance or visa which can be found in applicants passport or travel document?

What do i put here??

D8. give details of the journey , including where the applicant travelled from ,ticket booking or reference number, and transport operator,?

what do you suggest here?

D10 When did the applicant arrive in the UK.?

which date should i refer here?
