INDIFINITE LEAVE APPLICATION FORMAT

Hi

New appointment booking service format has changed.I am having a
difficulty in finding a correct form. I am under HSMP came to UK
Oct 2006.(Which is before November 2006.). I am highly greatfull, if someone let me know the correct format :!: and should i pay at the time of making application?

For ILR, SET(O) is the Form. You can pay the fee at the time of appointment at PEO. Or by credit/debit card, postal orders etc if you want to send it by post.
