documents required for tier 2 application

My company got the license last week. I am preparing to apply for my visa now. Could any one please tell me what are the documents required to submit with the aplication.

i have read the policy guidance. I am switching from Tier- 1 post study work visa and applying from inside the country .

Please let me know if they are correct.
1.6-months pay slip or 6-months bank statement.
2. degree certificate
3.maintenance of funds (if the company doesn't tick the box "that they will accomodate for first month")
4.Letter from the company confirming my name, duration and job title.

I am bit worried about the SOC codes. I am working as web developer and earning 30,600 for last six months, due to get a pay rise next month.
